Let the staff know before you are seen by the physician if you have a form or letter to be filled. There is a minimum $25.00 charge for all forms and letters, due at the time you request the form. Please provide all necessary information to be included and any details that will facilitate our office staff in helping you with the requested form or letter. Please allow at least 10 to 15 working days to process.
We've partnered with Physician's Hearing Network to bring to our patients FREE HEARING TEST. Please ask our front desk for more informtion.
For prescription refills please call your pharmacy directly. Your pharmacist will contact us for authorization. Please allow 2 to 3 business days to process your request. The prescription will be called directly to the pharmacy. Sometimes, your Health Care Professional might not authorize the refill, as he might want to review your symptoms and condition. This is for your own safety. There will be no prescriptions refilled during the weekend or after hours.
Please be aware that the Physician will not prescribe narcotics or controlled substances over the phone, on weekends or after hours. Narcotics and controlled substances require that you be seen for an appointment in the office. This is for your own protection.
We now offer telemedicine visits, depending on your insuance, o as a cash service for your convenience. You can be seen from the convenience of your home. Give us a call and we can arrange it for you.
If you need a referral, please call our office. It might be necessary to set up an appointment with your Health Care Professional to examine you before the referral is issued, as most insurance companies require us to do so. Please allow 5 to 10 business days to process your referral.
There are other services we provide, such as family consultations, which are NOT covered by insurances. If you choose to have those services, you will be responsible for those charges. For your convenience, we accept Visa, MC, cash or check.
